Use of up-to-date laboratory methods for timely and reliable detection of tuberculosis patients positive for M.tuberculosis allows early determination of patient category by clinicians, prescription of adequate chemotherapy regimens, shortening of time to sputum conversion, and as a result, improve treatment effectiveness and prevent the spread of drug-resistant strains of M.tuberculosis. We comparatively analyzed results of M.tuberculosis detection using single specimen subjected to the following tests: molecular genetic GeneXpert PCR analysis, fluorescence microscopy, Bactec-960 test system, and traditional culture test on solid media. Specimens from 154 patients were analyzed. Based on test results, it was concluded that a timely detection of infectious tuberculosis patients requires making use of all battery of available bacteriological methods

To assess the effectiveness microscopic methods used in the Phthisiatry Research-Practice Center, we comparatively studied the results of Ziehl-Neelsen microscopy, fluorescence microscopy and LED-microscopy in detecting acid-fast bacilli (AFB).We comparatively analyzed (using Ziehl-Neelsen microscopy) a total of 400 positive smears with AFB detected by fluorescence microscopy, and specimens from 648 patients treated at Pulmonology department of the Yakutsk City Clinical Hospital, who were tested by their clinical diagnostic laboratory as negative for AFB after triple Ziehl-Neelsen microscopy of intact sputum. For comparative analysis between fluorescence and LED microscopy, specimens from 1082 new pulmonary tuberculosis patients were studied

Presented is the description of laboratory service responsible for microbiological diagnosis of tuberculosis in Sakha Republic (Yakutia). We analyzed results of tuberculosis detection performed using microscopy with Ziehl-Neelsen staining in clinical diagnostic laboratories of the primary healthcare facilities, in 2005 and over the period from 2012 to 2016. For the purposes of comprehensive assessment of the effectiveness of microscopic detection of acid-fast mycobacteria, the process of deriving indicative criteria values was scrutinized. Employing the criteria analyzed, we assessed effectiveness and quality of laboratory work on microscopic detection of acid-fast mycobacteria, and level of organization of workflow during detection of tuberculosis patients in primary healthcare facilities, based on Ziehl-Neelsen microscopy

Analysis of trends in statistics for smear-positive patients (MTB+) over the period from 2011 to 2016 showed that number of registered MTB+ patients with active pulmonary tuberculosis had decreased during the study period, both among all cases, and among new cases. Nonetheless, the size of infection reservoir defined as the number of MTB+ patients remains at high levels. We observed high rates of prevalence for drug-resistance, including multidrug resistance and extensive drug resistance (MDR and XDR). Prevalence rates of tuberculosis cases with MDR in Yakutia in the years 2013 to 2015 were higher than the same rates over Russian Federation by factors of 1.7-1.8-1.6, but showed a trend to decrease compared to the rates in the Far East Federal District. High proportion of newly identified patients with MDR among patients with drug-resistant tuberculosis (up to 68.5%) suggests the emergence of a totally new-made tuberculosis infection reservoir with resistance to first-line drugs. The policy pursued and supported in the Sakha Republic (Yakutia) is aimed at strengthening laboratory capabilities, adopting novel diagnostic technologies, improving epidemiologic surveillance, while supporting the currently running state programs against tuberculosis, and considering the currently observed decrease in tuberculosis incidence, this could lead to beneficial effect on existing and future burden of drug-resistant tuberculosis

Multidrug-resistant tuberculosis without prior treatment (primary MDR-TB) is a major problem in the Russian Federation, and also in Eastern Siberia and Far East. Mycobacterium tuberculosis isolates from patients with primary Tb at two referral sites in Irkutsk oblast and the Sakha Republic (Yakutia) were tested for conventional susceptibility and genotyped using MIRU-12. Outcomes from primary MDR-TB were compared between sites. Phylogenetic reconstruction was used to determined basic epidemic genotypes with MDR. Primary MDR-TB was common in both regions and phylogenetic patterns; drug-resistance and outcomes carried regionally distinct differences, emphasizing the need for locally informed diagnosis and treatment strategies

The paper focuses on the substantiation of application of uniform generalizing criterion for a complex estimation of epidemiological situation and (or) population health state on example of epidemiological situation on tuberculosis in Republic Sakha (Yakutia) for 19 years (1990 - 2008). It is shown that in spite of multidirectional dynamics of analyzed certain statistical indices, generalized quantitative estimation of the epidemiological situatio

Epidemiologic characterization of tuberculosis in the Sakha Republic (Yakutia) over 38 years, on 4 main indicators: incidence, morbidity, sputum smear positivity, and mortality (1970-2007) is presented. In general, the named indicators are shown to have decreased markedly over the study period, owing to those years when the population had experienced a relative social well-being. But the last years (1990-2007) are characterized by a strong aggravation of epidemiologic situation

Purpose. Elaboration of intensive preoperative specific chemotherapy and determination of surgical treatment efficacy in patients with pulmonary tuberculesis with multiple drug resistance. Material and methods. As a clinical material for the survey 65 patients with multiple-drug-resistant tuberculesis were selected, who had been operated in the pulmonary-surgical department of the Yakutsk Research Institute of tuberculesis from 2000 to 2003. Treatment efficacy was being determined when observing patients in the course of 2-3 years after the operation. Results. In 76.6 % cases the long-lasting chemotherapy promotes to MDR tuberculesis forming and efficacy of the operation depends direct proportionally on duration of the disease. In 23.1 % patients with lower severity of MDR tuberculesis micobacterial tuberculesis (MBT) was a find being detected from the resected lung portion after operation. Surgical treatment efficacy of MDR tuberculesis is estimated at 83.0 %, postoperative complications - 10.7 %, postoperative activation of tuberculesis - 4.6 %, lethality - 6.1 %. Conclusion. Initial patients with prevalent MDR tuberculesis are to be operated on when the phase "treatment prolongation" expires, i.e. in the late 5-6 months of chemotherapy, others with lower severity - in 2-4 months
